NEW ATHLETIC FACILITY DESIGN!!!

Before you start practice for ANY team at Lynbrook High School. . .

Before starting practice for any athletic team, the Athletic/Activity Participation Authorization form must be completed and brought to the first tryout. This is your permission to start practice.

Athletic packets may be picked up at the administration office or the Athletics’ Office.

Be sure you have read and understand the entire athletic packet which includes:

  • F.U.H.S.D. Athletic/Activity Participation Authorization (Form 6510) as well as the two Pre-Participation forms to be signed by a parent, the student/athlete, and your physician.
  • F.U.H.S.D. Co-Curricular Code of Conduct and Steroid Contract

Once the physical form is completed, it is valid for the entire 09-10 school year. A copy will be kept on file in the Athletic Office for use by winter and spring sports.
